Terms and Conditions for NK Orthodontist Refer a Friend Program

1. Eligibility
   • The Refer a Friend program is open to current patients of NK Orthodontist.
   • Referrers must be 18 years or older to participate.
2. Referral Process
   • Referrers must obtain explicit consent from their friend before sharing their contact information with NK Orthodontist.
   • Referrals should only be made to close friends and family members.
   • NK Orthodontist will contact the referred friend to schedule a free initial consultation.
3. Reward
   • Both the referrer and the referred friend will receive a $150 credit if the referred friend begins comprehensive orthodontic treatment with NK Orthodontist.

   For the Referrer:

   • If the referrer has at least $150 still owing on their account, the $150 credit will be applied directly to their account balance.
   • If the referrer has less than $150 owing or has completed their payments, NK Orthodontist will provide them with a $150 VISA gift card.

   For the Referred Friend:

   • The $150 credit will be applied to the referred friend’s initial orthodontic treatment quote for comprehensive treatment.
   • This credit is valid for comprehensive orthodontic treatment only and does not apply to first-phase treatment, retainers, or limited treatment plans.

   General Conditions:

   • The credit cannot be redeemed for cash.
   • The credit will be applied after the referred friend has started their comprehensive treatment plan.
   • The $150 credit cannot be combined with other promotional offers unless explicitly stated.
4. Limitations
   • There is no limit to the number of friends a patient can refer.
   • The referral offer is valid for 6 months from the date of referral.
5. Privacy and Consent
   • By participating in this program, referrers confirm they have obtained consent from their friend to share their contact information.
   • NK Orthodontist will handle all personal information in accordance with our Privacy Policy and applicable privacy laws.
6. Communication
   • Referred friends will receive an initial contact from NK Orthodontist regarding the referral and free consultation offer.
   • Referred friends can opt-out of further communications at any time.
7. Program Changes
   • NK Orthodontist reserves the right to modify, suspend, or terminate the Refer a Friend program at any time without prior notice.
   • Any changes will not affect credits already earned for completed referrals.
8. Disqualification
   • NK Orthodontist reserves the right to disqualify any participant who violates these terms and conditions or engages in fraudulent activity.
9. Liability
   • NK Orthodontist is not responsible for any disputes between referrers and their referred friends regarding the program.
10. Acceptance of Terms
    • Participation in the Refer a Friend program constitutes acceptance of these terms and conditions.

By participating in the NK Orthodontist Refer a Friend program, you acknowledge that you have read, understood, and agree to these terms and conditions.
